Welcome to the PLT Nutrition Testimonial Video Contest, where we celebrate your remarkable journey to a healthier, happier life through our coaching and nutrition programs. By sharing your testimonial video, you not only inspire others but also stand a chance to win 6 months of free coaching with PLT Nutrition and a $50 gift card. We understand that entering may seem daunting, but every achievement, no matter how big or small, deserves recognition, and we're here to celebrate your success. By participating in the PLT Nutrition Testimonial Video Contest, you agree to comply with the following terms and conditions: 1. Eligibility: The contest is open to individuals who are clients of PLT Nutrition at the time of submission. Participants must be at least 18 years of age or older. Employees, affiliates, and immediate family members of PLT Nutrition are not eligible to participate. 2. Contest Period: The contest will commence on September 2023 and will continue until at least 50 valid testimonial video responses have been submitted. 3. Entry Submission: To enter the contest, participants must create a video testimonial sharing their journey and success with PLT Nutrition. Videos must adhere to the specified guidelines provided by PLT Nutrition, including but not limited to content, format, and duration. All entries must be submitted through the designated submission channel, which is Dropbox. 4. Prize Selection: The winner will be selected at random from eligible entries. Prize selection will take place no earlier than January 2024. The winner will be notified via the email address provided at the time of submission. 5. Prize Redemption: The winner must respond to the notification email within 14 days. To redeem the prize, the winner must confirm their intention to start the 6-month coaching program with PLT Nutrition within the specified 14-day period. This confirmation must be communicated via email to hr@pltnutrition.com. 6. Alternative Prize Option: If the winner is unable to start the coaching program within the 14-day period, they may choose an alternative prize option: A $100 PLT Nutrition gift card for personal use or gifting to a friend. A 25% discount on the next three consecutive months of coaching with PLT Nutrition. 7. Usage Rights and Release: By submitting a testimonial video, participants grant PLT Nutrition a perpetual, worldwide, non-exclusive, royalty-free license to use, reproduce, distribute, and publicly display the video for promotional purposes. Participants affirm that they have obtained all necessary permissions and consents from any individuals featured in their testimonial video. 8. Disqualification: PLT Nutrition reserves the right to disqualify any entries that do not comply with these terms and conditions or are deemed inappropriate. PLT Nutrition may disqualify participants who engage in fraudulent or unethical behavior related to the contest. 9. Technical Issues: PLT Nutrition is not responsible for any technical issues, including but not limited to, internet connectivity problems, hardware or software failures, or email delivery failures, that may affect entry submission or notification. 10. Privacy: Personal information collected during the contest will be used for the purpose of administering the contest and in accordance with PLT Nutrition's privacy policy. 11. Changes or Termination: PLT Nutrition reserves the right to modify, suspend, or terminate the contest at any time without notice. In the event of such actions, PLT Nutrition shall not be liable to participants. 12. Governing Law and Jurisdiction: These terms and conditions are governed by the laws of USA. Any disputes arising under or in connection with these terms and conditions shall be subject to the exclusive jurisdiction of the courts located within USA. 13. Entire Agreement: These terms and conditions constitute the entire agreement between participants and PLT Nutrition regarding the contest, superseding all prior and contemporaneous agreements, understandings, and representations.
